2. ⁠Appoint a proxy to collect on your behalf at your appointment time slot. You need to print and submit this authorization form for collection.

You may authorize someone to return the academic dress at the University on your behalf. The authorized person needs to provide the order number for verification.

When the rental academic dress is returned in good condition, the deposit will be refunded to you. If there are damages like iron marks, burnt marks, tears, decolourization found on the gown, hood, mortar board or co-curricular cord, the deposit will be forfeited to offset the cost of repair or replacement.
